CCI Creekside Care Services
Our program generates an individually tailored service plan that supports the resident’s own ability to perform activities of daily living, or ADLs. Services CCI can assist with may include the following:
- Complimentary care assessment
- Medication management
- Bathing, dressing, grooming, toileting, status checks
- Personal one-on-one care tailored to fit your needs
- À la carte and flexible care packages
- Low minimum requirements
- On-site presence
- 24-hour personal care available
Responding to Change
CCI’s training includes documenting services provided and noting trends that may demonstrate a change in needs. By meeting the changing needs of our residents by developing a service plan with low minimum requirements. CCI’s on-site presence gives residents the ability to have à la carte and flexible options, the availability of 24-hour care as their needs change, and allows for quick response to resident needs throughout the day.
Team Work
It’s a team effort that works best. We strive to work in partnership with residents, their families, physicians, and other health care professionals to maintain a service plan that addresses the needs of the resident.
